Your First Visit
Your first visit is an opportunity for us to get to know you, understand your concerns, and learn what is most important to you when it comes to your dental health. Your dentist will perform a comprehensive examination, including a thorough review of your medical and dental history and any necessary X-rays or diagnostic images. We will take the time to discuss our findings, answer your questions, and review any recommended treatment options. We believe you should be an active partner in your care, and we truly listen to and value your input when creating a treatment plan that feels right for you.
Please assist us by providing the following information at the time of your consultation:

Any x-rays taken within 1-5 years by a previous dentist.

If you have dental insurance, bring the necessary cards or policy and group number. This will save time and allow us to utilize your insurance benefits.
- A list of medications you are presently taking.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
